Position #1: Wellness Coach for the Well Balanced Program
Purpose:
This position will partner with an adult mental health consumer who is enrolled in the Well Balanced program, in a supportive relationship. Wellness Coach will work with individuals who are diagnosed with diabetes and support their efforts to keep their diabetes under control.
Key Relationships:
Well Balance program participants, project nurses, Compeer program coordinators and care coordinators.
Responsibilities:
- Attend any training provided by Monroe County Office of Mental Health that will give an overview of general health and also focus on diabetes.
- To be committed to wellness and healthy life styles.
- Work individually or with a small group of individuals who are enrolled in the Well Balance program to support them in their management of their diabetes through exercise, nutrition choices, assistance with doctors appointments, supporting them monitoring their medical status, reporting to the project purses, and any other activities that are appropriate and supportive.
- Collaborate with the individual’s care coordinator, project nurse, and Compeer program coordinator, while supporting the individual’s management of their diabetes.
- Act as an advocate for recipients enrolled in the Well Balance program when appropriate.
- Participate in the design and implementation of operational procedures that promote rehabilitation and recovery options for enrollees.
- Collect pertinent data and participate in an evaluation process to determine the strengths of the project.
- Recognize and value diversity within the work environment. Participate in valuing individual differences and supporting one another’s opportunity to achieve his or her greatest potential and be culturally responsive to the population being served.
Qualifications:
- Prior experience with mental health systems preferred.
- An understanding and appreciation of cultural competency in mental health service delivery.
- Reliable transportation and a good driving record are required.
Stipend: $10/hour plus gas mileage

Position #2: Mentor/Skill Builder - Community Connections for Youth Program
Key Responsibilities:
In conjunction with the youth and family team, to provide mentoring and skill building to youth with Serious Emotional Disturbances or who have a significant mental health concerns, for the successful completion of treatment plan goals. Using positive youth development philosophies, the aim is to mentor and provide skill building tools that will enable youth to be self-sufficient and confident in their home and in their community.
Job Tasks:
- Work individually with referred youth within the parameters of the therapist’s specified goals and Family Team Meeting Plan.
- Work with small groups of identified youth to foster socialization; skill development and personal growth utilizing your skills and agency resources.
- Develop relationship with youth's family to foster smooth transitions and positive communication.
- Coordinate with the youth, the youth’s family, therapist and other family resources in order to effectuate access to community resources and implementation of goals.
- Be available for treatment team meetings, if necessary, to report progress of goals.
- Keep ongoing progress notes as well as monthly reports. Must be able to submit notes and other pertinent information in a timely and accurate manner.
- Participate in program evaluation and improvement.
- Adhere to the policies and procedures this position requires.
- Attend employee meetings and trainings and other program related events as required.
- Demonstrate sensitivity to and respect for the diverse cultural and linguistic backgrounds of the youth and families served.
Education and Qualifications:
High School education or GED. Previous case management or experience working with youth having serious emotional difficulties preferred. Knowledge of mental health systems is also helpful. Must be reliable, consistent and able to work independently. Reliable transportation and good driving record is required.
Salary: $10/hour
Hours: Flexible: evening and weekends may be required, 3 hours / week / per youth

Position #3: Skill Builder, Supportive Partners for Recovery Program
Key Responsibilities:
To partner with an adult mental health consumer, who is enrolled in the Western New York Care Coordination Program,* in a supportive relationship. To pursue social and life enhancing skills that the consumer has identified in their recovery plans. The goal of the relationship is to provide support so the enrollee can access and be successful in community venues.
- Work individually with an adult mental health consumer who is enrolled in a WNYCC Program on a mutually agreed upon schedule towards an identified goal.
- Collaborate with the WNYCC Program’s Care Coordinator and the Compeer Program Coordinator to locate community venues and successfully implement the program.
- Work with the consumer to provide feedback to Compeer and the Care Coordinator on the outcome of the activities
- Participate in program evaluation and improvement.
- Attend client enrichment activities and other program related events as required.
- Demonstrate sensitivity to and respect for the diverse cultural backgrounds of the community.
Experience:
Previous work/volunteering with mental health consumers and knowledge of mental health systems preferred. Reliable transportation and good driving record are required.
Stipend: Approximately $10/hour plus gas mileage
Hours: Vary - up to 3 hours a week per consumer

* The Western New York Care Coordination Program is a collaborative initiative between six county governments, the New York State Office of Mental Health, providers and consumers, to transform community services systems serving people diagnosed with mental illness. The goal of the program is to create systems that are responsive to the interests of consumers, ensure access to high quality services and promote recovery. Service Delivery is based upon an individual services plan developed in partnerships with consumers and their families: A recovery system of care that is person centered and person driven.
